Treatment

For Mild Cases:

 

Tab Perinorm 10mg 1 tds (metochlopramide)

Tab. Emeset 4mg or Siquil 1 stat and S.O.S.

Gelusil MPS 2 tsp x 4 hourly (Antacid). If gastritis.

Ask the patient to chew cardamom, ginger, lime or crushed ice.

Tab. Gastractive 10 mg 3-4 times/ day ½ hour before meals (Domperidone). To normalise gastric motility.

If patient is taking drugs causing nausea e.g. flagyl, chloroquin then stop then stop the drugs and give injectible form.

 

For significant nausea or vomiting:

 

Inj. Perinorm 2cc I.M o I.V.

If not controlled, Inj. Emeset 4mg I.V. (Ondasteron).

Or. Inj. Siquil 2cc IM or I.V (Trifluopromazine)

IF associated pain or colic,

Inj. Anafortan 2cc IM (Antispasmodic)

If vomiting is prolonged and intake is poor: I.V. fluids – Ringer’s Lactate or DNS.

 

If extra pyramidal reaction occure.

To Siquil/ Stemetil/ Metochlopramid, with turning up of eye balls.

 

Tab. Pacitane 2 stat (Anti. Parkinson)

Tab. Calmpose 1 stat (Transquiliser)

Inj. Calmpose 2cc IM or IV or Inj. Phenergan 2cc IM or IV (Antihistaminic)

 

If vomiting is green coloured (Bilious), IF vomiting is associated with abdominal distension and colicky pains. Then suspect Intestinal obstruction and refer the patient to surgeon.

 

If vomitus/ patient’s breath has smell of Kerosene or Op.P Poison (like Tik 2.0), Refer to hospital for stomach wash and treatment.

 

In female patient, rule out early pregnancy.

 

If there is no nausea, but vomiting, headache and fever rule out meningitis.

General Measures Clear fluids orally, and small quantities of dry foods like biscuits
If severe, leading to dehydration and alkalosis, would need hospital admission for IV administration of saline and of potassium chloride.
If severe, nasogastric suction for decompression of stomach to improve patient comfort, and fluid intake and output have to be monitored.
   
Inadequate Response Hospitalize to ascertain case.
   
Prevention For motion sickness, and vomiting due to cancer chemotherapy, as detailed above
